Management:1. Supervise direct patient and family care team
members, in collaboration with program site leadership2.
Responsible for scheduling, payroll and other administrative
duties3. Ensure all initial assessments and required reassessments
are conducted by qualified team members within required time
frames, including documentation completion, submission and/or
synchronization4. Ensure the 24-hour availability of nursing and
psychosocial services to patients and families and coordinates
"on-call" assignments5. Oversee and support the Interdisciplinary
Group (IDG) in development, implementation and review of the
interdisciplinary plan of care for each patient and family6. Ensure
all patient and family services comply with federal, state(s) and
local regulatory requirements and with accepted professional
standards7. Collaborate with program site leadership members to
monitor, manage and mentor staff in patient care activities,
productivity, and documentation per related policies and
protocols8. Participate in the selection and evaluation of all
direct patient and family care team membersEducation9. Participates
and promote Back to Bedside efforts in reinforcing Case Management
skills, knowledge and attitude per protocol for all disciplines10.
Provides external education and in-services to community
partners11. Collaborate with program site leadership to support
staff education efforts12. Participate in orientation and ongoing
education of staff and volunteersInterdisciplinary Group13. IDG
Meeting Preparation: a. Prepares patient and care quality data
analysis (may include: Adverse Events, triage trending, med costs,
red/yellow patients, OA, Advance Directive issues etc.) to provide
timely staff support and feedback as necessary during and/or after
meeting b. May review above prepared data analysis with Associate
Team Director (ATD) prior to IDG Meeting and coordinate staff
support strategies14. Facilitates the IDG team meetings: a. Engages
and encourages all IDG participation b. Prepares and follows up on
identified patient care concerns (see prep above)15. Coordinates
with ATD/ Team Assistant (TA) to ensure regulatory documentation
requirements addressed as needed16. Ongoing IDG Support: Utilizes
proactive management strategies to assist staff, patients and their
families, which may include: a. Triages incoming calls during
business hours b. Provides caller support and assistance c.
Performs quality calls d. Documents all above per protocol e.
Facilitates CMR timely follow up and provides staff support in
promoting proactive care management practices17. Will assume
responsibility for direct patient care and Administrator-on-Call
(AOC) duties as requiredQuality18. Investigate (root cause
analysis) and follows up with staff to process issues in
collaboration with ATD and other program site leadership as needed,
and documents outcomes per protocol regarding: a. Adverse Events b.
Triage Trending results c. Quality of Care Calls d. Circle of Care
Calls e. Deyta / CAHPs and other quality measures / outcomes19.
Participate in or leads quality improvement activities, including
data collection and aggregation as able20. Participate in ongoing
quality assurance activities related to patient and family
services, including Quality Assurance & Performance Improvement
(QAPI) studies and chart audits to ensure the quality of patient
and family servicesCommunications21. Reinforces and supports all
internal and external communications related to: a.
Interdepartmental information b. Administrative process changes and
messaging c. Participate in the development, implementation and
evaluation of policies and procedures for patient and family
services d. Ensure all interdepartmental communication and exchange
of data is accurate, timely and effective e. Provide proactive
customer service maintenance22. Communicate as needed with members
of the team, the patient and their families, the Medical Director,
the Director-Clinical Operations and others involved in the care of
the patient to facilitate optimal care, especially during
transition situations23. Participate on committees and special
projects24. Performs other duties as assigned including those
within the scope of current, valid professional
licensure/credentials (i.e. RN, SW, Chaplain, MT, HA, etc.).

degree, preferred2. Registered Nurse license in state(s)
practicing, required3. Certified Hospice Nurse (CHPN), preferred4.
Current Cardiopulmonary Resuscitation (CPR) certification,
preferred a. Required in state of Georgia5. 3+ years' experience in
hospice, home health, general medical surgery, oncology or patient
population specific (i.e.: geriatrics, pediatric, etc.) nursing6.
Minimum of one year of supervisory experience7. Proficient in
Microsoft Office Suite of programs and EMR systems8. Must have
reliable automobile, valid driver's license, and the minimum state
required liability automobile insurance9. In addition to the above,
the highest clinical nursing position must possess the requirements
below:--- AZ: The clinical director shall be a registered nurse.
The state title for the clinical director is the Director of
Nursing.--- CA: The clinical director shall be either of the below.
The state title for the clinical director is the Director of
Patient Services. o A registered nurse with a baccalaureate or
higher degree in nursing or another health related field with three
years of experience within the last five years in a hospice or home
health agency, primary care clinic or health facility, at least one
year of which was in a supervisory or administrative capacity; or o
A registered nurse with four years' experience within the last five
years in a hospice, home health agency, primary care clinic or
health facility, at least one year of which was in a supervisory or
administrative capacity.--- CO: The clinical director shall be a
registered nurse employed by the hospice. The state title for the
clinical director is the Director of Nursing--- CT: The clinical
director shall be a registered nurse with baccalaureate degree in
nursing and an active Connecticut license, and who is further
qualified by one of the below. The state title for the clinical
director is the Director of Hospice Patient Care Services.o A
master's degree from a program approved by the National League of
Nursing or the American Public Health Association with not less
than two years' full-time clinical experience under qualified
supervision, in a hospice or home health care agency related
community health program that included care of the sick; oro Not
less than four years of full-time clinical experience in nursing,
at least two of which were under qualified supervision in a hospice
or home health care agency or community health program that
included care of the sick.--- DE: The clinical director shall be
licensed in the State of Delaware. The state title for the clinical
director is the Director of Nurses.--- FL: The clinical director
shall be a registered nurse. The state title for the clinical
director is the Supervising Registered Nurse.--- GA: The clinical
director shall be a registered nurse. The state title for the
clinical director is the Director of Nursing.--- IN: The clinical
director shall be a registered nurse. The state title for the
clinical director is the Patient Family Care Coordinator.--- MD:
The clinical director shall be a registered nurse with training or
experience in hospice nursing. The state title for the clinical
director is the Registered Nurse Supervisor.--- MA: The clinical
director shall be a health care professional possessing academic
training and experience in direct patient care and shall be
qualified to coordinate the clinical services provided by the
hospice. A staff person serving in another position within the
hospice may also be the coordinator of clinical services if he/she
meets the requirements for both positions and can adequately carry
out the duties of both positions. The state title for the clinical
director is the Clinical Services Coordinator.--- MI: The clinical
director shall be registered and licensed in the State of Michigan.
The state title for the clinical director is the Director of
Nursing.--- MO: The clinical director shall be employed by the
hospice. The state title for the clinical director is the
Registered Nurse Coordinator.--- NV: Nursing care must be provided
by a registered nurse or under the supervision of a registered
nurse.--- NJ: The clinical director shall be either:o A registered
professional nurse who has at least one of the following
qualifications. The state title for the clinical director in this
case is the Director of Nursing.? A master's degree in nursing or a
health-related field and two years combined public/community health
nursing and progressive management experience in public health
nursing; or? A Bachelor of Science degree in nursing or a
health-related field and three years combined public/community
health nursing and progressive management experience in public
health nursing.o A registered professional nurse who has at least
one of the following qualifications. The state title for the
clinical director in this case is the Nursing Supervisor.? A
Bachelor of Science degree in nursing and two years combined public
health nursing and progressive professional responsibilities in
public health nursing; or? Three years combined public health
nursing and progressive professional responsibilities in public
health nursing.--- OR: The clinical director shall be a registered
nurse. The state title for the clinical director is the Registered
Nurse Coordinator.--- PA: The clinical director shall be a
registered nurse and a graduate of a school of professional
nursing.--- TX: The clinical director shall be a registered nurse
licensed in Texas or in accordance with the Texas Board of Nursing
rules for Nurse Licensure Compact and have at least one year of
experience as a registered nurse within the last 36 months. The
state title for the clinical director is the Supervising Nurse.---
