Job Description:

A company is looking for a Care Manager (RN).

Key Responsibilities:

Evaluates member needs and develops care plans focusing on member priorities
Identifies barriers to care and provides appropriate interventions
Coordinates with providers and resources to ensure members receive necessary care

Required Qualifications:

Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
2-4 years of related experience
Active RN - Registered Nurse - State Licensure or Compact State Licensure
