competent, inclusive care. Responsibilities 1. Assess patient and
family for physical, psychological, social and spiritual needs and
collaborates with patients and families, physician, and
Interdisciplinary Group (IDG) to establish the plan of care,
implement interventions and/or evaluate care2. Initiate and
maintain communication with attending physician, Medical Director,
IDG, Team Director (TD), and IDG and other agencies to coordinate
optimal care and use of resources for the patient and family a.
Especially, in the event of unusual or potentially problematic
patient and family issues3. Document according to protocol and
maintain up-to-date patient records a. Ensure plan of care
problems, interventions, actions and goals reflect baseline,
current, and ongoing status, and changes as they occur4.
Participate in On-Call or Admissions assignment/rotation, as
directed5. Provide education to patient and caregiver(s) as needed,
based on knowledge needs and learning assessment, patient and
family condition, in keeping with plan of care6. Coordinate care
with IDG based on the determined Plan of Care7. Participate in
orientation and ongoing education programs, on committees and
special projects and other related duties as assigned8. If serving
as a Care Manager; a. Proactively manage using the Case Management
Recipe to coordinate all patient care activities in collaboration
with IDG b. Scheduling patient visits in keeping with needs,
physician collaboration and orders management, medication and
orders management, medication management, plan of care
coordination, etc. c. Ensure continuity and quality of care and
coordinated IDG efforts d. Supervise Licensed Practical Nurses,
Licensed Vocational Nurses and Hospice Aides per protocol e.
Participate in IDG Meetings, facility patient conferences and other
patient and family meetings in coordination with IDG and/or Team
Director9. If serving in an On-Call capacity; a. Perform
Admissions, GIP, CC and routine visits, as assigned b. Respond to
Triage Nurse to receive patient status information and coordinate
follow up including visit activity, coordinating medication refills
and physician orders and phone support c. Coordinate schedule and
care activities with Administrator On Call as necessary d. May
participate in IDG Meetings, facilitate patient conferences, and
other patient and families meetings in coordination with IDG and/or
Team Director e. The employee is free to spend their time as they
choose, until they become engaged in work10. Performs other duties
as assigned including those within the scope of current, valid
professional licensure/credentials (i.e. RN, SW, Chaplain, MT, HA,

Qualifications1. Associate degree, preferred2. Licensed as a
Registered Nurse (RN) in state(s) practicing3. Current
Cardiopulmonary Resuscitation (CPR) certification, preferred a.
Direct Patient Care in Georgia and California, required4. Previous
experience in hospice, home health, general medical surgical or
patient population specific (i.e., geriatrics, pediatric, etc.)
nursing5. Willingness to serve on-call6. Must have reliable
automobile, valid drive's license and the minimum state required
liability auto insurance7. Excellent written and verbal
communication and organizational skills8. Working knowledge of
computer programs, especially Microsoft Office Suite products and
EMR systems9. Ability to work as part of an interdisciplinary
team10. Meet the regulations and requirements of the State in which
