Job Description:

Seeking a PRN Nurse Practitioner for home risk assessments, welcoming new grads.
Conduct comprehensive assessments, including medical history, diagnosis, treatment, and social factors, during visits up to one hour.

Collaborate with physicians, case managers, and caregivers to improve patient care and close care gaps.

Provide services in home, skilled facilities, and community settings with flexible hours-----"days, evenings, nights, weekends.

Requirements include a Master--------s degree or equivalent, current RN and NP licensure, certification in relevant specialties, BLS/ACLS, and at least 1-year NP experience.

Must have travel ability, a valid driver--------s license, and vehicle insurance. Compensation ranges from $2,200 to $11,000/month based on assessments completed. Benefits include mileage reimbursement and limited voluntary benefits.
