required Description: Wed/Thu/Fri shifts available. On-call
required. Local candidates accepted within 90 miles. Flu vaccine
required, with medical or religious exemptions only. Requests for
time off can be made for no more than five non-consecutive days.
During the holiday season (November to January), only one holiday
request can be made. Approval for time off is required prior to
signing a contract. A state-issued ID is required prior to
submission. Modules are required and must be completed before
starting. Any modules not completed prior will be finished during
orientation and annually. Module hours vary by specialty.
