Job Description:

The primary role of the Social Work Supervisor is to plan, organize, and direct the work of professionally trained social workers who are responsible for assessing the psychosocial needs of members and providing risk-focused care coordination to address issues that impede their ability to adhere to their medical care plan.* Improve workflows regarding social work referral processes specific to meet the need of the local market.* Monitor referrals and case management activities.* Provide individual supervision to assigned social work staff.* Respond to requests for consultation to assigned social workers as needed.* Identify, track, resolve and report any quality of care issues.* Participate as an active member of the local interdisciplinary care team (IDT).* Meet and confer with market leadership regarding local issues, including patient care plans.* Participate in interviewing, hiring, evaluating and disciplining of social worker staff.* Assist with education, in-service training and social work orientation.* Provide input regarding quarterly performance evaluation of assigned social work staff.* Participate in resolution of conflicts between social workers and among social workers, members, and Landmark Health personnel.* Participate in local leadership meetings.* Prepare for and lead regular meetings with social work team.* Develop and maintain productive relationships with key community partners.* Ensure that a method is in place/followed to update community resource list for all areas served by the market.* Provide social work coverage based on market needs.* May provide clinical supervision to associate clinical social workers.* Travel to patient's homes to provide services.* Minimum of 5 years of social work experience (internship not included) that includes direct case management and/or clinical work with patients.* Master's Degree (MSW) in Social Work* Experience working in or with healthcare* Excellent communication skills and knowledge of accessing community resources* Two years of social work supervisory experience required in some markets, preferred in others* Experience in delivering home-care or field case management is preferred* Two years as a Licensed Clinical Social Worker (LCSW) required* Possession of a valid state driver's license and access to reliable transportation to conduct patient visits requiredIn order to work with potentially vulnerable patient populations and to keep employees healthy while caring for patients, all patient-facing employees of Landmark must provide evidence of the following required immunizations/vaccinations as a condition of employment: TB, MMR, Tdap, and Influenza.
